Welcome, New Patients!

We are a primary care practice and family doctor accepting new patients from the Exton, Downingtown, West Chester, Coatesville, Chester Springs, Malvern, and other Chester and Delaware County areas. Adults ages 16 and up who are looking to establish a plan of care and have regular visits with a primary care practice are welcomed to choose a clinician from our team and enroll as a new patient.

The first step

Do you accept my insurance?

We accept most major insurances. We do not accept Medicaid, nor do we enroll private/cash-pay patients (paying entirely out of pocket). We are not in-network with many EPO (Exclusive Provider Organization) and multi-plan insurances. We accept patients for whom we are out-of-network, but your copays may be higher. Before completing your New Patient paperwork to enroll as a patient, please call your insurance company to be sure we are in-network or affordable to you out-of-network.

Your Medical Records

Your next step is to request that your previous doctor forward any and all relevant medical history and records to CMMD & Associates so we may properly complete your chart.

It is your responsibility to ensure that we receive these records. If you have hard copies, they can be mailed or faxed to us; alternately, your clinician may transmit them to newpatients@christinemeyermd.com using a secure, HIPAA-protected email account (do not send them using a consumer email account such as GMail or Yahoo, as this is not secure).

After scheduling your first appointment, you will be directed to complete our comprehensive New Patient paperwork. We offer a completely online version, or a PDF that can be mailed or emailed. The form will take some time to complete, but go slow and tell us your full health story – we're listening.

How do we do things?

When it comes to our patients, we don't just want an appointment – we want a relationship! We want to know you and your health story, so that we can provide the personalized care and proactive medical advice that will keep you well throughout your adult life stages.

Our relationship will consist of sick visits for things like colds, flu, rashes, or new aches and pains. We'll also see you regularly for chronic conditions like diabetes, hypertension, or STDs. We also ask all of our patients to have a well visit once per year. Also known as a physical, we review your vitals, new general concerns, medications and specialists, and other things you might not bring up when you're here for a cough or cold. This visit also lets us provide referrals if needed.

Now, every good partnership has two parties – that means we also need you! As your new primary care provider, we ask that you:​

  • Call us first before seeing a specialist, so that we can save you time, copays, and get you to the right specialist.

  • Call us for non-emergencies, instead of going to the ER. With your help, we can keep ERs from being overwhelmed and healthcare costs from ticking upward. We have appointments seven days a week, and until 8pm on weekdays for things like injuries, pain, and illness.

  • Keep us informed of ER visits, hospital stays, surgeries, tests, or specialist visits. Call us right away after being in the hospital. Keeping your primary care doctor aware of your complete health picture is one of the best ways to ensure great healthcare across your conditions or concerns.

Clinicians and Care Teams

Our practice has a skilled team of physicians, CRNPs and PA-Cs who can see you for well and sick visits. Each clinician is a part of a Care Team, a cluster of clinicians in our office who coordinate and consult with each other. Whenever possible, we'll schedule you with a clinician on your primary clinician's care team. But, any clinician on our team is accessible to you at any time, particularly if your clinician does not have availability right away.

The Patient Portal

We use a state-of-the-art online patient portal called MyPennChart. After your first appointment, you will receive an invitation to join the portal and set up your account. Learn more on our portal account page here.

Your First Visit

Please plan to arrive 20 minutes early to your appointment for first-time registration and check-in. You should have already completed your New Patient Paperwork (online or as a PDF you can bring with you).​

All patients must bring their current insurance card, photo ID, and a payment method (cash, credit card or check) for their copay. Copays are due at the time of your appointment.

If you are more than 10 minutes late for your scheduled appointment time, the appointment may need to be rescheduled. Please plan ahead for traffic in our area!
